Toulon Rehabilitation & Health Care Center

My mother is at Toulon Rhabilitation & Health Care Center and the staff treat her like she is family. The care is excellent. The staff are like my family to me now. The doctors are very caring and everything is taken care of. She eats healthy (they have good tasty food). She is very happy there. Thank you everyone!

Team Toulon (the staff) is awesome! The caring, hardworking people that work at this facility are very competent and kind. They work through issues that arise in a professional and common sense way. The facility really needs updates in furnishings, remodeling, medical equipment for nurses and aids and amenities. It also seems there is a little shortage in staff at times, but the staff present just works harder. There needs to be hearing services that comes to the facility. Food is mostly good and well balanced taking care to meet the requirements of each resident. My loved one is well cared for here.

Wish i could give them 0 stars my mom was in their facility not even a full 24 hrs and she was found unresponsive and rushed to the er and put in icu, due to their staffs negligence. They knew they didnt have 24 hr respiratory staff, nor did anyone have training on how to care for patients with tracheas. My mom was a nonverbal patient who required someone to keep eyes on her due to her not being able to vocalize when something didn’t feel right and they had her in a room with no machines to monitor her heart rate,or the proper ventilator for her trachea. As I stated before my mom was a nonverbal patient, they were not checking on her frequently ( as they should since she had no one in the room to monitor her) so they didnt even know how long she was under my mom end up suffering, severe brain trauma and when we were notified, they told us she was down for 5 to 10 minutes but when we got to the emergency room they told er staff she was down for 20 to 25 minutes and we also found out her trachea was dislodged. If you want your family member to live and get better please don’t send them here. Thanks to them and their negligence I have no mother, but they will be getting served soon!

I have contacted Toulon Health Care 5 times regarding a small bill my father received. Each time I call I am treated as if what I am saying means nothing. I have not received a call back and there has been no response to a letter I wrote 6 months ago. Additionally the bank confirmed the check cleared! Obviously it was credited to someone else’s account or maybe someone else’s pocket. If you are looking for a place with good financial skills this may not be for you.

I Love Toulon Rehab! I Enjoy working on our Alzheimer Dementia Unit. The staff gives great care to our residents!
